Collaborates with other HBPC staff, educating the veterans and caregivers regarding the proper use of medications. Provides anticoagulation management for all applicable patients enrolled in HBPC. Identifies patient-specific medication issues, including drug interactions, adverse effects, lack of effectiveness, appropriateness, and compliance problems. Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations as necessary.
